Registration for Hospital Procedures
Pre-registration for surgery
For day surgery and planned
surgical admissions, Davis Memorial Hospital offers the convenience
of preadmission.
Preadmission and preoperative evaluations allow our nurses to collect information, perform tests and explain the surgery process to the patient. They are conducted prior to the day of scheduled surgery. Plan to be at the hospital for three or four hours in order to complete all aspects of your preadmission process.
It is helpful if patients bring information regarding allergies and a current medications list to the preadmission interview.

for outpatient, emergency, inpatient and day surgery registration.
